Clean Initiative Report (Charter/Form View)

Due to the way the Initiative page is set-up, there is no professional way to report on Initiatives as a business case or project charter directly from Aha!. We've tried to manipulate a pivot report but due to current limitations, it doesn't allow you to visualize field names in the left most column while having content in the cells (I've gone through support to confirm). 

We are looking for a clean, charter-like form that can be downloaded & shared (see attached).
