Add an index to notebooks Merged

When sharing notebooks that are more than a few pages long it would be very helpful to have the option to include a generated index allowing stakeholders to easily navigate to the specific page(s) that are important to them.

Users can create their own index today, but links to different pages within the notebook open up new tabs each time.
