Allow choice of form for idea creation via Zendesk integration

When creating a new Idea from Zendesk using the Aha! Zendesk integration the form only contains a product selection, name and description for the Idea. We have custom fields we capture on new ideas using a custom Aha! form and it would be good to have the option to select this form to be used for idea creation from within zendesk.
