Buttons not visible for small screens
I don't use a monitor and have a 13" laptop. A lot of your forms, this one included, aren't accessible for me as most of the buttons are cut off.
Ask: designs consider smaller screen sizes

Yes! There are things I literally cannot do on my laptop because the submit (or whatever its called) button is off the bottom of my screen.
