Custom Fields - display descriptions when displayed on forms above field rather than in help bubble

In order for the descriptions of custom fields to appear in collection forms, we have had to implement complex JavaScript (second screen shot), because otherwise, the descriptions in the custom fields do not persist and are only available when hovering over the help bubble. The reason this is an issue is that the user may need to reference information in the description while typing their response.
